Under the supervision of the Program Lead or Program Manager, the Intensive Care Facilitator is responsible for the coordination, scheduling and facilitation of Child and Family Team (CFT) meetings. The Intensive Care Facilitator will conduct regular CFT meetings for each client on their caseload within the timeframe identified by the program and/or needs of the client/family. They will complete associated documentation including the Safety Plan, Outcome Measures, CFT Matrix and progress note. This position will collaborate with inter and intra-agency providers to coordinate care, monitor and ensure resources and linkages are accessible and appropriate for the youth and family. They are well versed in and are able to communicate to others the program philosophy, principles, and process. Services are provided primarily in the field including at clients’ residence, school, other community location, or in the office. The Intensive Care Facilitator is responsible for completing all required documentation in accordance with Agency, County, and COA standards.
