About The Position

Matter Health is seeking compassionate, dependable Care Facilitators for a seasonal, part-time, field-based role supporting older adults in affordable housing communities. Care Facilitators travel to assigned residential communities and help patients connect with Matter Health providers through virtual visits. They greet patients at scheduled appointment times, assist with consent forms on a company-provided iPad, connect patients to their providers, and help ensure visits run smoothly. This is a non-clinical support position: Care Facilitators do not provide medical care, make clinical decisions, conduct patient outreach, or build a patient panel. The role requires local daily travel between assigned communities using a personal vehicle. Employees work onsite at residential communities, follow a structured schedule, and communicate delays or issues to the Market Operations Leader. The work may involve frequent walking, standing, stairs, and carrying company equipment weighing up to 10 pounds. The seasonal assignment's duration depends on operational needs and may be extended or ended according to the needs of the initial patient visit program. Compensation is $17.00–$20.00 per hour. The role is not eligible for Matter Health's standard full-time employee benefits package.
