About The Position

The Behavioral Health Clinician (BHC) is an integral member of the primary care team, working collaboratively and consultatively to improve patients’ overall health and well-being. The clinician provides early identification, assessment, triage, and evidence-based, problem-focused interventions for behavioral, emotional, psychological, and social concerns related to physical and mental health conditions. The BHC meets individually with patients to conduct comprehensive assessments and makes recommendations to primary care providers to enhance patient care plans. When appropriate, the clinician provides follow-up sessions involving skills training, coaching, and evidence-based interventions to improve health, functioning, and wellness. The role also helps patients transition to community-based behavioral health resources when their needs exceed services available in the clinic.
