About The Position

The Behavioral Health Clinician (BHC) is an integral member of the primary care team, working collaboratively and in a consultative role to improve patients’ health and well-being. The clinician provides early identification, assessment, triage, and evidence-based, problem-focused interventions for behavioral, emotional, psychological, and social concerns related to physical and mental health conditions. The BHC meets individually with patients to conduct comprehensive assessments and recommends ways to enhance their care plans in close collaboration with primary care providers. When appropriate, the clinician provides follow-up treatment through skills training, coaching, and evidence-based interventions to improve patients’ health, functioning, and wellness. The position also helps patients transition smoothly to community-based behavioral health services when their needs exceed the services available in the clinic.
