Legacy Health•Mount Angel, OR
•$43 - $64

About The Position

The Behavioral Health Clinician (BHC) is an integral member of the primary care team, working collaboratively and consultatively to improve patients’ health and well-being. The clinician provides early identification, assessment, triage, and evidence-based, problem-focused interventions for behavioral, emotional, psychological, and social concerns related to physical and mental health conditions. The BHC meets with patients individually to conduct comprehensive assessments and recommends ways to enhance their care plans in collaboration with primary care providers. When appropriate, the clinician provides follow-up treatment through skills training, coaching, and evidence-based interventions to improve health, functioning, and wellness. The role also supports patients who need services beyond those available in the clinic by facilitating transitions to community-based behavioral health resources. This position calls for advanced skill in evidence-based interventions for a range of emotional and behavioral problems. The BHC works with patients and the primary care team to coordinate patient-centered care, and may provide several subsequent sessions of skills training and coaching when indicated.
