Requirements
- Valid NY State RN license with a BSN degree
- Proven experience in Intake or CHHA roles
- Strong knowledge of the Home Care Homebase (HCHB) system
- Familiarity with Allscripts and Careport platforms
- Excellent communication and organizational skills
- Compassionate approach to patient care and coordination

- Experience with Intake procedures
