Requirements
- Valid New York State RN license.
- BSN degree.
- Proven experience in Intake or CHHA roles.
- Strong knowledge of the Home Care Homebase (HCHB) system.
- Familiarity with Allscripts and Careport platforms.
- Excellent communication and organizational skills.
- Compassionate approach to patient care and coordination.
