This role involves providing comprehensive case management services to individuals experiencing chronic homelessness. The specialist will be responsible for managing an assigned caseload, conducting regular client meetings (both in the field and office), and ensuring all clinical files and reports are accurate and up-to-date. Key responsibilities include conducting thorough assessments to identify and help clients overcome barriers to housing, assisting them in securing entitlements, developing and implementing individualized housing plans, and building a network of support services. The specialist will also prepare clients for housing interviews, arrange appointments, and participate in case conferences. A crucial aspect of the role is liaising with clinical staff from other BRC or external programs regarding shared clients. Fire Safety and First Aid/CPR certification, or the ability to obtain them, is required. Related duties may also be assigned.
