This role is responsible for managing a caseload of single men and women transitioning into housing after experiencing homelessness. Clients will have a mental health diagnosis (SPMI) and/or substance use disorder. The specialist will assist with intake, admissions, entitlement applications and re-certifications, develop and update service plans, maintain chart documentation, provide supportive counseling, and facilitate support groups. They will also teach and assist with daily living activities, money management, rent collection, and recreational activities. Additionally, the role involves connecting clients with community resources for medical, mental health, and substance abuse services, utilizing a harm reduction approach and motivational interviewing techniques. Regular apartment visits are required to ensure client safety and compliance with city guidelines. The specialist will participate in community and interdisciplinary team meetings, be on call during off-hours, and attend BRC-approved trainings. Emergency first aid/CPR assistance may be needed. This position reports to the Program Supervisor.
