In-Home Physician Full Time - La Crosse
About The Position
Signify Health, part of CVS Health, is seeking a full-time physician to provide in-home health evaluations and help bridge gaps in care. Physicians visit people in their homes to build trusted relationships and provide flexible, comprehensive, and personalized health evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and, when ordered by the person’s health plan, additional diagnostic tests such as diabetic eye exams and spirometry. Physicians use an iPad and an intuitive clinical workflow. The role does not require prescribing medicine, ordering lab tests, or changing a person’s current treatment regimen. The role may require local travel and travel within licensed states, as well as virtual visits. The primary workplace is the private residence of health plan members. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants; clinicians must be able to perform their duties comfortably and safely in these conditions. Full-time employees qualify for a comprehensive benefits package. The anticipated schedule is 40 hours per week. The typical pay range is $174,070 to $374,920 annually; actual salary depends on factors including experience, education, and geography.
Requirements
- An active, unrestricted physician license in the coverage area or areas.
- Ability to comfortably and safely perform duties in members’ homes, which may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities
- Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
- Provide additional Diagnostic & Preventive Services when indicated, such as spirometry, diabetic eye exams, and bone density screening.
- Conduct virtual visits as needed.
- Travel locally and within licensed states as needed.
- Fulfill the requirements of the specified clinician role and provide Signify Health services as indicated.
- Help connect people with appropriate care and support improved health outcomes.
