About The Position

Signify Health, part of CVS Health, is seeking a per diem Nurse Practitioner to provide In-Home Health Evaluations in members’ homes and help bridge gaps in care. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or change a member’s current treatment regimen. The role includes local travel and travel within licensed states as needed, virtual visits as needed, and delivery of Signify Health services, including Diagnostic & Preventive Services products. The primary workplace is members’ private residences, which may contain pets, secondhand smoke, or other airborne allergens or irritants; clinicians must be able to perform their duties safely and comfortably in these environments. This part-time role is anticipated to involve 12 hours per week. Per diem employees receive an hourly base rate plus an additional incentive structure; malpractice insurance coverage during Signify Health-related activities; supplies; and other perks. The stated pay range is $37.66–$81.11 per hour. Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.
• Ability to comfortably and safely perform duties in members’ homes, which may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

• Conduct In-Home Health Evaluations, including medication and medical history reviews and physical evaluations.
• Perform additional diagnostic tests when ordered by the member’s health plan, such as diabetic eye exams or spirometry.
• Use an iPad and the clinical workflow for each evaluation.
• Build trusted relationships with members, answer health questions, and help connect them with appropriate care.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
