About The Position

Signify Health, part of CVS Health, is seeking a per diem Nurse Practitioner to provide in-home health evaluations and help bridge gaps in care. The role focuses on building trusted relationships and providing flexible, comprehensive, and personalized health evaluations in members’ homes. Visits last approximately 45 minutes and include time for one-on-one discussion, a medication and medical history review, and a physical evaluation. Additional diagnostic tests, such as diabetic eye exams or spirometry, may be included when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order laboratory tests, or change a member’s current treatment regimen.

The position involves local travel and travel within licensed states as needed, as well as virtual visits when needed. The primary workplace is members’ private residences, which may include household pets, secondhand smoke, and other potential airborne allergens or irritants. Per diem employees receive an hourly base rate with possible additional incentives, malpractice insurance coverage for Signify Health-related activities, supplies, and other perks. This part-time position is eligible for benefits and programs depending on hours worked and eligibility, including dental and vision coverage, wellness resources, employee discounts, and access to certain voluntary benefits.

Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Connect one-on-one with members, answer their health questions, and help guide them toward better health.
• Provide connections to appropriate care based on members’ individual needs.
• Use an iPad and clinical workflow to complete evaluations.
• Provide indicated Signify Health services, including diagnostic and preventive services such as spirometry and diabetic eye exams.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Fulfill the requirements of the specified clinician role.
