About The Position

OCES is growing and expanding its services to 11 more towns in the South Shore area. OCES supports older adults and individuals with disabilities by providing vital information and coordination of services to promote healthy, safe living, allowing them to stay active and engaged in their communities. The organization is seeking a dedicated and caring Housing Specialist to work within the Housing team, providing services and support to residents on-site at South Shore area locations. This position is based in Stoughton but will also involve working at congregate sites in Braintree, Bridgewater, Hingham, and Marshfield.

Requirements

• At least two years of experience in the human services field, preferably with the elder population.
• Excellent written and verbal skills.
• Excellent computer skills.
• Proficient with Microsoft Office Suite or related software.

• Bachelor's degree in social work or related field preferred.
• Bilingual a plus.

Responsibilities

• Review the needs of residents to link them with community programs and services.
• Serve as a multi-disciplinary assessment team leader and meet with the team to assure service coordination.
• Facilitate care conferences with residents, family, and other participants.
• Plan and coordinate social, educational, and recreational programs for residents.
• Prepare and distribute a newsletter to keep residents informed of planned activities.
