Housing Specialist - Stoughton
About The Position
OCES is growing and expanding its services to 11 more towns in the South Shore area, including Quincy, Braintree, and Weymouth. OCES is dedicated to supporting older adults and individuals with disabilities by providing essential information and coordinating services to promote healthy, safe living, enabling them to remain active in their communities. We are seeking a compassionate and dedicated Housing Specialist to join our Housing team. This role involves working within the community and on-site at our South Shore locations to provide services and support to residents. The position is primarily based in Stoughton but will also involve work at congregate sites in Braintree, Bridgewater, Hingham, and Marshfield.
Requirements
- At least two years of experience in the human services field, preferably with the elder population.
- Excellent written and verbal skills.
- Excellent computer skills.
- Proficient with Microsoft Office Suite or related software.
Nice To Haves
- Bachelor's degree in social work or related field preferred.
- Bilingual a plus.
Responsibilities
- Review the needs of residents to link them with community programs and services.
- Serve as a multi-disciplinary assessment team leader and collaborate with the team to ensure service coordination.
- Facilitate care conferences involving residents, families, and other participants.
- Plan and coordinate social, educational, and recreational programs for residents.
- Prepare and distribute a newsletter to inform residents about planned activities.
