This full-time position will be based at our housing site in Collinsville, IL. Requirements

High school diploma or equivalent and five years of supervised mental health experience.
Alternatively, a bachelor’s degree in counseling, social work, education, psychology, or a related human service field is required.
Individuals with a bachelor’s degree in any other field must have at least 2 years of supervised clinical experience in a mental health setting.
Demonstrated skills in human services and in services to adults.
Demonstrated ability to work within organizational structure; to accept supervision; and to work constructively with consumers, treatment resources, and the community.
Basic computer skills including MS Word and an electronic medical record.
Valid driver’s license, private auto insurance, and be insurable.

Responsibilities

Provide services where they are most effective—meeting clients in their homes or other community settings.
Offer initial triage and support during crisis situations, ensuring a rapid and grounded response.
Utilize the IM-CANS assessment tool to co-create consumer-driven, personalized care plans.
Ensure all identified services meet program requirements while serving as a roadmap for the client's specific recovery goals.
Conduct individual, family, and group therapy sessions using Evidence-Based Practices (EBPs).
Act as a dedicated advocate by providing case management, medication training, and consistent monitoring.
Help clients navigate the complexities of the healthcare system and understand their treatment regimens.
Manage the essential "behind-the-scenes" continuity of care, including the pre-authorization and re-authorization of services to ensure treatment remains uninterrupted.
