This full-time position will be based at our housing site in Collinsville, IL. Requirements

  • High school diploma or equivalent and five years of supervised mental health experience.
  • Alternatively, a bachelor’s degree in counseling, social work, education, psychology, or a related human service field is required.
  • Individuals with a bachelor’s degree in any other field must have at least 2 years of supervised clinical experience in a mental health setting.
  • Demonstrated skills in human services and in services to adults.
  • Demonstrated ability to work within organizational structure; to accept supervision; and to work constructively with consumers, treatment resources, and the community.
  • Basic computer skills including MS Word and an electronic medical record.
  • Valid driver’s license, private auto insurance, and be insurable.

Responsibilities

  • Provide services where they are most effective—meeting clients in their homes or other community settings.
  • Offer initial triage and support during crisis situations, ensuring a rapid and grounded response.
  • Utilize the IM-CANS assessment tool to co-create consumer-driven, personalized care plans.
  • Ensure all identified services meet program requirements while serving as a roadmap for the client's specific recovery goals.
  • Conduct individual, family, and group therapy sessions using Evidence-Based Practices (EBPs).
  • Act as a dedicated advocate by providing case management, medication training, and consistent monitoring.
  • Help clients navigate the complexities of the healthcare system and understand their treatment regimens.
  • Manage the essential "behind-the-scenes" continuity of care, including the pre-authorization and re-authorization of services to ensure treatment remains uninterrupted.
