About The Position

The Housing Services Lead provides subject matter expertise and operational support to housing services staff to promote effective, consistent program procedures and compliance with administrative procedures and corporate policies. The role also coordinates and provides supportive services to individuals and families in the assigned housing program, including applications, outreach, home visits, social service referrals, crisis intervention, case management, and client advocacy. The Lead performs the duties of the corresponding Housing Services role, supports assigned personnel, leads staff meetings, and consults on participant housing service plans. The position includes assessing participant needs, developing and monitoring service plans, coordinating referrals and community linkages, supporting staff across clinic locations, and contributing to training and onboarding. The Lead also assists with outreach initiatives, patient engagement projects, housing referrals, funder and organizational reporting, and monitoring team productivity. The position is supervised and evaluated by Behavioral Health Leadership. Requirements

Three years of post-secondary education, or three years of experience in lieu of education.
Experience working with underserved populations.
Knowledge of, commitment to, and sensitivity toward people experiencing housing insecurity, substance misuse, mental health challenges, or other challenging circumstances.
Working knowledge of social service resources in the service area for patients with behavioral health issues, including city, county, state, and federal resources.
Ability to work independently.
Excellent communication and customer service skills.

Responsibilities

Perform the duties of the Housing Services job description corresponding to staff on the team.
Support assigned Housing Services personnel in meeting the requirements of their respective job descriptions.
Lead staff meetings and provide consultation regarding individual participant housing service plans.
Assess participant needs and develop, implement, and monitor service plans.
Assist participants in meeting service plan goals and objectives.
Maintain collaborative linkages with health care providers, social service agencies, shelters, housing services, and other appropriate providers.
Coordinate and support Housing Services staff at clinic locations to ensure policies and procedures are followed.
Develop training aids and provide onboarding support for the Housing Services team.
Assist the Clinical Supervisor or Associate Behavioral Health Director with organizational community, outreach, and in-reach initiatives.
Work with Housing Support leadership to monitor team productivity.
Work with organizational leaders on projects to increase patient engagement.
Manage housing referrals that require follow-up.
Assist with completing and submitting monthly reports required by funders and Terry Reilly Health Services.
Continuously expand knowledge of community resources and services.
Work collaboratively with Housing Services staff, support positive relationships with participants, and provide feedback to team members.
Build and maintain positive working relationships with other providers.
Meet regularly with the supervisor and/or Associate Behavioral Health Director to review activities, identify problems, and establish priorities.
Participate in staff, clinic, corporate, and team meetings as directed by the supervisor.
May participate in personnel evaluations of assigned case management staff and provide feedback to leadership.
Assist in developing additional services with supervisor approval.
