About The Position

Lenox Hill Neighborhood House, a leading human services provider founded in 1894, offers a wide range of integrated services including social, educational, health, housing, mental health, nutritional, and fitness programs to approximately 16,000 individuals annually on Manhattan's East Side. Our clients span all age groups and include older adults, unhoused individuals, children and families, recent immigrants, persons with disabilities, and adult learners. The Women's Mental Health Shelter, located in the Park Avenue Armory, is a 24-hour program dedicated to assisting 80 homeless women, aged 45 and over, who have a history of mental illness. Through a comprehensive continuum of care, including social work, medical, nursing, psychiatric care, peer support, housing placement, benefits assistance, arts, recreation, and nutrition programs, our multidisciplinary team helps clients transition from homelessness to permanent housing. The Housing Placement Coordinator plays a crucial role in implementing our individualized mental health model to foster long-term stability and secure permanent housing for clients. This role requires a commitment to community-based human services with a social justice perspective, supporting clients in optimizing their wellness and achieving housing stability. The coordinator will manage the entire housing placement process from intake to successful move-out.

Requirements

  • Minimum of three years' experience working directly with clients in relevant fields, including homelessness, chronic mental illness, substance abuse, trauma, and/or residential services.
  • Excellent oral and written communication skills.
  • Excellent organizational skills.
  • Excellent emotional intelligence.
  • Leadership, engagement, and interpersonal skills to effectively collaborate with a range of key stakeholders and community providers.
  • Flexibility and availability to support program operations as needed, given the 24/7 operating schedule and client needs.
  • Must be able to participate in the on-call rotation and provide operational support when coverage needs arise.

Nice To Haves

  • Bachelor's degree with relevant experience in homelessness services, behavioral health, substance use services, or residential support.
  • Spanish fluency.

Responsibilities

  • Manage the housing process for all clients, including understanding available housing options, recommending best fits, and ensuring successful completion and approval of housing applications (e.g., 2010E applications, HPD Section 8, NYCHA Section 8, CityFHEPs vouchers).
  • Provide direct client services, including assessments, case management, escorts, housing-related support and educational groups, follow-ups, and referrals to resources.
  • Maintain an active caseload, completing needs assessments and bi-weekly Individualized Living Plans (ILPs), and providing case management and housing support for stability and permanent housing.
  • Coordinate the entire housing process and lifecycle from intake to stabilization and placement, including completing 2010E applications, voucher eligibility forms, housing applications, and assisting with housing interviews.
  • Assist with treatment planning and implementation, case conferences, and weekly clinical and housing staff meetings, collaborating with on-site psychiatric and medical providers.
  • Help clients achieve individual stability, obtain housing, and develop skills to ensure they remain permanently housed.
  • Maintain and ensure timely management of client records, housing packages, data collection, and project completion.
  • Liaise with governmental partners, community-based landlords, and other housing providers to track, monitor, and implement successful housing outcomes.
  • Collaborate with Lenox Hill Neighborhood House staff from multiple departments and disciplines, as well as other community partners, to maximize support and program offerings.
  • Complete all other responsibilities and duties as assigned by the Director.
