About The Position

Northeast Counseling Services is seeking a Housing Case Manager to join its Forensic Assertive Community Treatment (FACT) Team. The position provides community-based rehabilitation and support services through intensive outreach to help individuals achieve stability, independence, and recovery. Working as part of the FACT Team, the Housing Case Manager assists individuals involved with the criminal justice system in locating, securing, and maintaining safe, stable housing. The role provides case management, advocacy, resource coordination, and rehabilitation services while supporting clients’ treatment and recovery goals. The position requires successful completion of an FBI Clearance, PA Criminal Clearance, and Child Abuse Clearance.

Requirements

Bachelor's degree in Social Work, Psychology, Counseling, Rehabilitation Counseling, or a related field from an accredited college or university.
Valid Pennsylvania Driver's License.
Ability to work independently and as part of a multidisciplinary team.
Strong communication, organizational, and problem-solving skills.
Successful completion of an FBI Clearance, PA Criminal Clearance, and Child Abuse Clearance.

Nice To Haves

Experience working in behavioral health, case management, housing services, or community-based programs.

Responsibilities

Provide community-based case management and rehabilitation services.
Assist clients in locating, obtaining, and maintaining appropriate housing.
Coordinate services with landlords, community agencies, treatment providers, and other support systems.
Monitor client progress and document services in accordance with agency and regulatory requirements.
Participate in multidisciplinary FACT Team meetings and treatment planning.
Assist clients in accessing benefits, community resources, and supportive services.
Maintain regular contact with clients through outreach in home and community settings.
Support clients in developing independent living skills and recovery goals.
Perform other related duties as assigned.
