About The Position

The Medical House Calls program is specifically tailored to support elderly patients in their desire to age comfortably at home, aiming to minimize hospital readmissions and overall healthcare costs. This initiative is driven by an interdisciplinary team that delivers comprehensive primary care directly to patients in their homes or assisted living facilities. Typically serving patients aged 65 and above who face challenges in attending traditional medical appointments, the program primarily assists individuals managing multiple chronic conditions, coping with changes in functional ability, handling polypharmacy, and dealing with frailty or debility. The primary care services provided through this program involve intensive patient management and meticulous care coordination, with a particular focus on advance care planning. To ensure optimal patient outcomes, the program team establishes robust partnerships with various healthcare entities, including inpatient teams, hospice agencies, skilled nursing facilities, physical therapy, occupational therapy, mental health services, and county support services. Requirements

  • Masters in Advanced Clinical Practice or graduate of a post masters certificate program for nurse practitioners, OR certificate from an education program that is accredited by the Council of Nurse Anesthesia Educational Programs/Schools, American College of Nurse Midwives, American Association of Colleges of Nursing of the National League for Nursing. For physician Assistants, graduate of an accredited Physician Assistant program and certification by National Commission on Certification of Physician Assistants.
  • No experience required.
  • Basic Life Support (BLS)
  • Licensed Physician Assistant or Nurse Practitioner
  • If Nurse Practitioner: RN Compact License(depending on State of primary residence) AND a license (or eligibility for) as a nurse practitioner or physician assistant in the Commonwealth of Virginia AND a "Practice Agreement" signed by sponsoring/ supervising physician(s), AND a written "Scope of Practice/Protocol" signed by the supervising physician, AND "Drug Enforcement Administration License" (if prescribing controlled substances)
  • Ability to successfully complete the credentialing process by third-party payors and Ability to enrolled as a CMS Medicare/Medicaid payor
  • Successful completion of credentialing for Medical Staff membership and appropriate clinical privileges sufficient to perform all required services

Responsibilities

  • Provide safe and effective primary care in collaborating with the interdisciplinary Medical House Calls team.
  • Utilizes independent judgment in the assessment, diagnosis, treatment planning, and provision of ongoing care to ambulatory patients that is in accordance with nurse practitioner licensure regulations for the Commonwealth of Virginia and delineated in the written Scope of Practice/Protocol for Advanced Practice Professionals (Licensed Independent Practitioners).
  • Demonstrates ability to communicate effectively and work collaboratively with others.
  • Exhibits respect for and understanding of other clinical disciplines and uses an integrated approach to achieve desired patient outcomes.
  • Uses evidence-based standards, guidelines, and protocols to the delivery of patient care.
  • Incorporates new information and approaches, when appropriate, to continuously improve patient care and practice.
  • Ensures ongoing growth and development of clinical skills and knowledge by assuming responsibility for lifelong learning.
  • Provides direction and guidance to others regarding practice, also serves as a resource and mentor.
  • Demonstrates leadership skills through participation in program development.
  • Works in collaboration with the Physicians attending to provide care in Primary Medical.
  • Management of various Care Plans.
  • Performs new consultations, patient follow ups, daily rounding and procedure triaging.
  • Writes/ dictates history and physical notes, consult, and daily notes.
  • Prescribes/ orders medications, therapies, diet, orders and consistent with clinical privileges.
  • Ensure key quality metrics are optimized.
  • Participates in quality improvement projects as assigned.
  • Accurate and complete billing for services provided.
