About The Position
Join our busy, well-established Heart Failure Clinic. As a key member of our team, you’ll manage complex heart failure patients, design comprehensive treatment plans, and drive optimal clinical outcomes. We are looking for a compassionate, hardworking NP or PA with a strong cardiology foundation and exceptional knowledge of advanced heart failure care.
Requirements
- Active Nurse Practitioner (NP) or Physician Assistant (PA) certification.
- Minimum 2+ years of outpatient cardiology experience.
- Strong clinical knowledge of complex cardiac conditions.
- Exceptional communication skills.
- Compassionate, patient-centered, and collaborative mindset.
Nice To Haves
- Prior experience in Heart Failure management.
Responsibilities
- Perform comprehensive assessments and diagnostic evaluations in an outpatient clinic setting.
- Develop, implement, and adjust individualized pharmacological and lifestyle treatment plans.
- Order and interpret echos, EKGs, and lab work.
- Work alongside cardiologists, nurses, and multidisciplinary teams.
- Guide patients and families on HF management and medication adherence.
- Support inpatient HF consults (as needed) and contribute to exciting program initiatives (e.g., palliative care collaboration, specialized amyloid/sarcoid clinics).
