About The Position
Community Options, Inc., a national non-profit agency providing services to individuals with disabilities in 11 states, is seeking a Host Home / Family Model Provider in the Cookeville, Tennessee area. The provider opens their home to an individual with developmental and/or physical disabilities and provides a 24/7 family-like setting. The role involves providing a safe, comfortable home and ensuring the individual’s basic physical and emotional needs are met so they can live a high-quality life. The provider must establish a comfortable, supportive, and genuine relationship with the individual they house and support. This is an independently contracted position. Providers are reimbursed at a per-diem rate and receive monthly room and board from the individual they support; the posting states this income is tax exempt.
Requirements
- High school diploma or GED.
- Valid driver's license.
- Previous experience working as a Direct Support Professional is required.
- Successfully complete trainings through Community Options.
- Be open to supervisory visits in the home.
