About The Position

The Hospitalist Care Facilitator assesses, plans, implements, and coordinates the discharge plan as directed by Hospitalist Program to maximize Hospitalist's efficiency. This role maximizes the efficient utilization of resources.

Requirements

  • Has completed the basic professional curricula of a school of nursing as approved and verified by a state board of nursing, and holds or is entitled to hold a diploma or degree therefrom.
  • Valid multi-state or State of Oklahoma Registered Nurse License.
  • Minimum 5 years of related experience.
  • Working knowledge of general disease state management processes, bio-psychosocial model, and clinical pathways.
  • Effective interpersonal, written, and verbal communication skills.
  • Ability to organize and prioritize work in an effective and efficient manner.
  • Ability to utilize PC & software skills.

Responsibilities

  • Assists with improving patient flow by expediting discharge orders and fielding questions/issues via telephone, secure chat and inbox messaging that needs to be discussed with physicians.
  • Consults and collaborates with treatment team (Hospitalists, Physicians, Advanced Practice Providers, Registered Nurse Case Managers, Social Workers, Educators, and/or other healthcare personnel) as appropriate to effectively coordinate patient discharge as ordered by the attending physician.
  • Participates in providing outstanding customer service and accepts responsibility for maintaining relationships that are equally respectful to all.
  • Collaborates with the Hospitalists and the hospital interdisciplinary care team to coordinate and manage the process of patient care.
  • Oversees the assimilation of information related to assessment, planning, implementation, and evaluation of patient needs to maximize the efficient utilization of resources.
