Position Summary: Working in an assigned territory or facility, facilitates referrals to agency by gathering and interpreting medical and financial data, meeting with patients and families and working with case managers to ensure optimal transition from hospital or other facilities to home. Responsible for promoting teamwork with all members of the healthcare team. Performs duties in a manner consistent with St. Luke’s mission and values.
