Position Summary: Working in an assigned territory or facility, facilitates referrals to agency by gathering and interpreting medical and financial data, meeting with patients and families and working with case managers to ensure optimal transition from hospital or other facilities to home. Responsible for promoting teamwork with all members of the healthcare team. Performs duties in a manner consistent with St. Luke’s mission and values.

Requirements

Graduate of an accredited nursing school or an approved school of social work with a MSW required.
Two years of experience working in an acute care setting
Current license (RN, LPN, LVN) to practice profession in the state of Missouri. Licensure not required for social workers.
Current driver’s license in state of residence.
Possession of a registered and insured motor vehicle.

Nice To Haves

one year of home care and/or hospice experience preferred.
