About The Position
The Hospice Team Leader oversees an interdisciplinary team including Registered Nurse Case Managers (RNCMs), social workers, and chaplains to direct and deliver high-quality hospice care within home and community settings. This role balances team management, staff development, administrative leadership, and direct patient care.
Requirements
- Current RN license
- Minimum of 3 years of hospice with supervisory experience
- Valid driver's license and insured, reliable vehicle
Responsibilities
- Enhance communication to ensure existing RN Case Managers, Social Workers and Chaplains receive support in the field.
- Conduct weekly IDT meetings for their designated team.
- Guide new staff with new employee orientation and education in hospice care.
- Perform timely performance check-ins with their team on a quarterly basis.
- Participate in ride-a-long visits with their team on a quarterly basis and as needed.
- Maintain a small caseload.
- Completes an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs.
- Participate in on call coverage.
