About The Position

Bridges Hospice is hiring multiple Registered Nurse Case Managers to provide hospice nursing care in assigned areas throughout the Pittsburgh region. This is a full-time, day-shift position based at the listed Monroeville, Pennsylvania location, with travel of up to 100%. Requirements

• Active Pennsylvania RN license in good standing.
• Strong clinical, communication, and organizational skills.
• Compassionate, dependable, and team-oriented approach.
• Valid driver’s license, reliable vehicle, and auto insurance.
• Ability to travel within the assigned territory.
• Ability to perform essential physical job duties, including lifting and assisting with patient transfers.

Nice To Haves

• One year of hospice nursing experience is preferred.

Responsibilities

• Provide compassionate, patient-centered hospice nursing care.
• Assess and manage patients’ pain and symptoms.
• Develop and coordinate individualized care plans.
• Educate and support patients and their families.
• Collaborate with the interdisciplinary hospice team.
• Document hospice eligibility.
• Supervise hospice aides.
