About The Position
Roze Room is a leading provider of Hospice and Palliative Care, celebrating 25 years of service to Southern California communities. This is a Full Time Field Hospice Registered Nurse (RN) position serving the Long Beach/South Bay areas, which also includes some Orange County and LA areas. This role matters because it involves changing lives at the most meaningful moments. As a Hospice RN Case Manager, you will guide patients and families through one of life’s most vulnerable transitions, delivering comfort, dignity, and compassionate care at home. Requirements
- Licensed Registered Nurse (RN)
Nice To Haves
- BSN/MSN (preferred)
- Experienced in hospice, home health, or case management (preferred)
- Compassionate, independent, and confident in the field
- Comfortable with EMR/documentation
Responsibilities
- Lead and coordinate individualized hospice care plans
- Provide skilled nursing care including pain and symptom management
- Supervise LVNs and CHHAs in the field
- Collaborate with physicians and the interdisciplinary team
- Educate and support patients and families during critical moments
- Make a real impact every single day
