About The Position

Hospice Nurses care for patients at the end of their lives, helping them live as comfortably and independently as possible and with the least amount of pain. They provide emotional support to patients and families as they face death, and care for patients’ minds, bodies, and spirits while supporting their families and caregivers. This role serves the Indianapolis Hospice House and Outpatient Hospice and requires travel of up to 75%.

Requirements

Associate degree in Nursing/Patient Care.
Registered Nurse (RN) license from the state licensing board.
Cardiopulmonary Resuscitation (CPR) certification from the American Heart Association.
Valid driver's license.
Auto insurance.

Nice To Haves

Bachelor's degree in Nursing/Patient Care.
One year of home health or hospice care experience.

Responsibilities

Determine patient eligibility for home care and hospice care.
Provide emotional, psychological, and spiritual support to patients, friends, and families.
Collaborate with patients, families, and multidisciplinary team members to support optimal patient outcomes and enhance the patient experience.
Assess, collect, and analyze objective, subjective, psychosocial, and cultural data relevant to patient condition, nursing diagnosis, needs, and plan of care.
Develop, review, and revise plans of care based on patient data, observations, and communications with patients and families.
Prioritize and implement the plan of care, coordinate care delivery, and employ interventions to improve patient outcomes while maintaining a safe, respectful, and ethical practice environment.
Communicate timely changes in patient assessments or reassessments to providers when they warrant consideration or a change in the plan of care.
Coordinate communication with multidisciplinary team members to facilitate safe transitions and continuity of care.
Promote patient independence by establishing patient care goals.
