About The Position

James River Hospice is seeking a compassionate and committed Hospice RN to provide intermittent hospice case management for assigned patients in their residences and coordinate with the interdisciplinary team to support an optimal patient-care experience. The role is based in Richmond, Virginia, and requires travel to Lynchburg as needed. The schedule runs from Friday at 5:00 PM through Monday at 8:00 AM.

Requirements

• Current Registered Nurse license to practice nursing in the Commonwealth of Virginia.
• At least one year of hospice experience.
• Graduate of an accredited nursing school.
• Valid driver's license and reliable car available for use during work hours.
• Must be willing to travel to Lynchburg as needed.

Responsibilities

• Complete an initial assessment of the patient and family to determine hospice needs.
• Develop a care plan with goals based on nursing diagnoses and palliative nursing actions.
• Coordinate with the clinical team supporting the patient and supervise LPNs and hospice aides involved in patient care.
• Spend time with patients to assist with pain management and provide support.
• Modify and reevaluate the plan of care when necessary.
• Monitor, record, and report symptoms and changes in the patient's condition.
• Obtain and receive physician orders as required.
• Participate in on-call duties as defined by the on-call policy.
• Perform other duties as assigned.
