About The Position

James River Hospice is seeking a compassionate, committed Hospice RN to provide intermittent hospice case management for assigned patients in their places of residence. The nurse coordinates with the interdisciplinary team to support an optimal patient-care experience. The role is based in Richmond, Virginia, with travel to Lynchburg as needed. The schedule runs from Friday at 5:00 PM through Monday at 8:00 AM. The salary range is $85,000–$105,000.

Requirements

Current Registered Nurse license to practice in the Commonwealth of Virginia.
At least one year of hospice experience.
Graduation from an accredited nursing school.
Valid driver’s license and a reliable car available for use during work hours.

Responsibilities

Complete initial assessments of patients and families to determine hospice needs.
Develop care plans that establish goals based on nursing diagnoses and incorporate palliative nursing actions.
Coordinate with the clinical team and supervise LPNs and hospice aides involved in patient support.
Spend time with patients to assist with pain management and provide support.
Modify and reevaluate care plans when necessary.
Monitor, record, and report symptoms and changes in patients’ conditions.
Obtain and receive physician orders as required.
Participate in on-call duties as defined by the on-call policy.
Perform other duties as assigned.
