About The Position

Mission Healthcare is hiring a Hospice RN Case Manager to provide patient- and family-centered nursing care in South Orange County, California. The role includes assessing patient and family needs, developing and updating individualized plans of care, providing prescribed treatments and medications, counseling patients and families, and supporting discharge planning. The position is full-time, Monday through Friday, from 8:00 a.m. to 5:00 p.m. The hourly pay range is $58.00–$62.00, depending on experience.

Requirements

• Current and valid state RN license.
• At least 1 year of recent acute care experience in an institutional setting.
• Basic Life Support (BLS) certification with hands-on CPR.
• Valid driver’s license and auto insurance.

Nice To Haves

• Bachelor’s degree in Nursing.
• 1 year of home health experience.

Responsibilities

• Complete initial assessments of patients and families to determine home care needs, including a physical assessment and history of current and previous illnesses.
• Initiate and revise the plan of care as the patient’s status and needs change.
• Regularly reevaluate patients’ needs.
• Use health assessment data to determine nursing diagnoses.
• Develop care plans with goals based on nursing diagnoses and therapeutic, preventive, and rehabilitative nursing actions, involving the patient and family in the planning process.
• Initiate appropriate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by the physician.
• Counsel patients and families on meeting the plan of care and related needs.
• Provide health care instructions to patients as appropriate to the assessment and plan.
• Identify discharge planning needs as part of care plan development and implement them before discharge.
