About The Position

Suncrest At Brighton/Suncrest Hospice aims to provide exceptional hospice care and service by increasing staffing resources, clinical visits, and staff-to-patient ratios while lowering clinician caseloads. The organization is a Community Health Accreditation Partner (CHAP)-certified hospice.

The Registered Resource Nurse provides care and services to a group of patients by visiting them where they reside, including private homes, skilled nursing facilities, and assisted living facilities. The nurse is a contributing member of the interdisciplinary team, offering expertise in psychosocial assessments, interventions, and community resources. The schedule is 2 PM to 10 PM.

Requirements

• Current Registered Nurse (RN) licensure in good standing in the state(s) of practice.
• Ability to work collaboratively within a team environment.
• Ability to work independently on a day-to-day basis.
• Time-management skills to coordinate visits, documentation, meetings, and urgent needs.
• Ability to learn an electronic medical records system.
• Willingness to drive, with reliable transportation, a valid driver's license, and auto insurance.
• Understanding of hospice rules and regulations.

Responsibilities

• Provide care and services to a group of patients.
• Visit patients where they reside, including private homes, skilled nursing facilities, and assisted living facilities.
• Contribute to the interdisciplinary team by providing expertise regarding psychosocial assessments, interventions, and community resources.
• Collaborate with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Manage visits, documentation, meetings, and urgent needs by prioritizing and triaging multiple concerns.
• Communicate by phone, email, and in writing; attend meetings with healthcare professionals.
• Travel across the service area to visit patients.
