About The Position

Requirements

Current RN license specific to the state or states assigned for work.
Current CPR certification.
Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

Performs initial nursing assessments for new admissions.
Completes, coordinates, and revises the total plan of care and maintains continuity of patient care by collaborating with appropriate staff.
Regularly communicates patient progress to the clinical manager, physician, and care team.
Develops, prepares, and maintains individualized patient care progress records accurately, promptly, and according to care center policies.
Records pain and symptom management changes and outcomes as appropriate.
Educates families about the patient’s condition, needs, and care.
Makes referrals to other hospice care disciplines as needed, or documents the rationale for not doing so.
Performs on-call responsibilities and provides on-call services to patients and families as assigned.
Performs other duties as assigned.
