About The Position

Amedisys is seeking a per diem hospice registered nurse to provide patient care in the home. The role includes assessing new admissions, coordinating individualized plans of care, communicating with the clinical manager, physician, and care team, and educating families about patients’ conditions and care needs. The nurse also documents patient progress and pain or symptom management, makes referrals to other hospice disciplines when appropriate, and provides assigned on-call services. Compensation is $40–$42 per hour plus mileage and may vary by location, job-related knowledge, skills, and experience.

Requirements

• Current RN license specific to the state or states assigned.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.

Nice To Haves

• One year of nursing experience preferred. Candidates with less than one year of nursing experience may be considered for the RN new graduate program.

Responsibilities

• Perform initial nursing assessments for new admissions.
• Complete, coordinate, and revise the total plan of care, maintaining continuity of patient care through collaboration with appropriate staff.
• Regularly communicate patient progress to the clinical manager, physician, and care team.
• Develop, prepare, and maintain individualized patient care progress records accurately and on time, in accordance with care center policies.
• Record pain and symptom management changes and outcomes as appropriate.
• Educate families about the patient's condition, needs, and care.
• Make referrals to other hospice care disciplines as needed, or document the rationale for not doing so.
• Perform assigned on-call responsibilities and provide on-call services to patients and families.
• Perform other duties as assigned.
