Registered Nurse Hospice Per Diem

About The Position

Amedisys is hiring a hospice registered nurse to provide patient care in the home. This per-diem role offers flexible hours and an hourly rate of $39–$42 plus mileage. The nurse performs admission assessments, coordinates and updates individualized plans of care, monitors and records patient progress and symptom management, and educates families. The role also includes assigned on-call responsibilities and other duties as assigned.

Requirements

Current RN license specific to the state(s) where assigned to work.
Current CPR certification.
Valid driver's license, reliable transportation, and liability insurance.

Nice To Haves

One year of nursing experience preferred. Candidates with less than one year of nursing experience may be considered for the RN new graduate program.

Responsibilities

Perform initial nursing assessments for new admissions.
Complete, coordinate, and revise the total plan of care, maintaining continuity by collaborating with appropriate staff.
Regularly communicate patient progress to the clinical manager, physician, and care team.
Develop, prepare, and maintain accurate and timely individualized patient care progress records in accordance with care center policies.
Record changes and outcomes related to pain and symptom management as appropriate.
Educate families about the patient's condition, needs, and care.
Refer patients to other hospice care disciplines as needed, or document the rationale for not doing so.
Perform on-call responsibilities and provide on-call services to patients and families as assigned.
Perform other duties as assigned.
