About The Position
As a per diem RN Case Manager, you will coordinate compassionate, individualized hospice care by assessing patients, partnering with the interdisciplinary care team, and developing care plans that support each patient’s needs. The service area includes Boothbay and surrounding coastal areas. The position includes on-call responsibilities and services for patients and families as assigned. Candidates must pass a drug test before beginning employment.
Requirements
- Current RN license specific to the state(s) assigned for work.
- Current CPR certification.
- At least 1 year of nursing experience.
- Valid driver's license, access to reliable transportation, and liability insurance.
- Must pass a drug test before beginning employment.
Responsibilities
- Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
- Make referrals to other hospice care disciplines as needed, or document the rationale for not doing so.
- Develop, prepare, and maintain accurate and timely individualized patient care progress records according to care center policies.
- Record pain and symptom management changes and outcomes as appropriate.
- Teach patients, families, and caregivers appropriate patient care techniques.
- Communicate regularly with the clinical manager and care team.
- Perform on-call responsibilities and provide on-call services to patients and families as assigned.
- Perform other duties as assigned.
