As a home care registered nurse, you provide comfort-focused care to hospice patients with prognoses of six months or less and build compassionate, meaningful connections with patients and the people in their lives. You work with an interdisciplinary team of hospice clinicians and advocate for patients and families, helping ensure their goals of care are communicated and prioritized. You also provide basic hospice education and training to patients, families, and caregivers. Most care takes place where patients call home, including private residences and assisted living or skilled nursing facilities. You typically make 4–6 visits each day and visit the local program office once a week to collaborate with an interdisciplinary team that includes social workers, physicians, chaplains, and aides.
