Hospice Registered Nurse (RN)
About The Position
As a home care registered nurse, you will provide comfort-focused care to hospice patients with prognoses of six months or less and build compassionate, meaningful connections with patients and the people in their lives. You will work with an interdisciplinary team of hospice clinicians, advocate for patients and families so their care goals are communicated and prioritized, and provide basic hospice education and training to patients, families, and caregivers. Most of your time will be spent caring for patients wherever they call home, including private residences and assisted living or skilled nursing facilities. You will typically make 4–6 visits each day. Once a week, you will visit the local program office to meet with the interdisciplinary team, which includes social workers, physicians, chaplains, and aides, to coordinate holistic support for patients and families. Requirements
- Associate or bachelor's degree in nursing.
- Valid RN license.
- At least one year of hospital nursing experience.
- Reliable transportation.
Responsibilities

- Deliver basic hospice education and training to patients, families, and caregivers.
- Make typically 4–6 visits each day in private residences or assisted living/skilled nursing facilities.
- Visit the local program office once a week to meet with the interdisciplinary team.
