About The Position

Residential Home Health and Hospice is hiring a Registered Nurse for a hospice position providing end-of-life care to patients in their homes in the Elmhurst, Illinois area. The role offers autonomy, meaningful patient relationships, and career growth, and includes eligibility for a $10,000 first-year success bonus tied to key milestones. The nurse works independently while collaborating with an interdisciplinary team focused on patients’ comfort, dignity, and quality of life. The field-based role involves travel to patient homes within an assigned territory.

Requirements

Active Registered Nurse (RN) license in the state where the employee works.
Two years of RN experience and clinical expertise to deliver quality patient care.
Valid driver’s license.
Ability to spend approximately 20% of the workday driving to and from patient locations.
Successful completion of a drug screen prior to employment.
Must be legally authorized to work in the United States.

Responsibilities

Deliver compassionate, patient-centered care in a hospice setting in collaboration with an interdisciplinary team.
Perform comprehensive patient assessments, symptom and pain management, and individualized end-of-life care planning.
Educate and support patients and families through the end-of-life journey.
Think critically and solve problems in real time.
Complete thorough, compliant EMR documentation.
Travel to patient homes within the assigned territory.
