About The Position

The Registered Nurse (RN) provides care and services to a group of hospice patients, visiting them where they reside, including private homes, skilled nursing facilities, and assisted living facilities. As a contributing member of the interdisciplinary team, the RN provides expertise in psychosocial assessments and interventions and knowledge of community resources. The RN works in a Case Management, Admissions, or Resource role.

Requirements

• Current Registered Nurse (RN) licensure in good standing in the state or states of practice.
• Ability to work well in a team environment and collaborate with colleagues.
• Ability to work one-on-one with patients while receiving support from a patient care team.
• Ability to work independently on a day-to-day basis.
• Time-management skills to manage visits, documentation, meetings, and urgent needs.
• Ability to learn an electronic medical records system.
• Willingness to drive and travel across the service area, with reliable transportation, a valid driver’s license, and auto insurance.

Nice To Haves

• Availability to work Tuesday through Saturday, 8:00 a.m. to 4:30 p.m.

Responsibilities

• Provide care and services to a group of hospice patients.
• Visit patients in private homes, skilled nursing facilities, and assisted living facilities.
• Contribute to the interdisciplinary team by providing expertise regarding psychosocial assessments, interventions, and community resources.
• Manage visits, documentation, and meetings; respond to urgent needs by prioritizing and triaging multiple concerns.
• Collaborate with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Communicate by phone, email, and in writing, and meet with healthcare professionals.
