About The Position

James River Hospice is seeking a compassionate, committed Hospice Registered Nurse to provide intermittent hospice case management for assigned patients in their places of residence in Lynchburg, Virginia, and surrounding counties. The nurse coordinates with the interdisciplinary team to support an optimal patient-care experience. The schedule is Monday through Friday, 5:00 PM to 8:00 AM. The posted salary range is $72,000–$93,000.

Requirements

Hold a current Registered Nurse license to practice in the Commonwealth of Virginia.
Have at least one year of hospice experience.
Graduate from an accredited nursing school.
Have a valid driver's license and a reliable car available for use during work hours.

Responsibilities

Complete an initial assessment of the patient and family to determine hospice needs.
Develop a care plan with goals based on nursing diagnoses and palliative nursing actions.
Coordinate with the clinical team supporting the patient and supervise involved LPNs and hospice aides.
Spend time with patients to assist with pain management and provide support.
Modify and reevaluate the plan of care as necessary.
Monitor, record, and report symptoms and changes in the patient's condition.
Obtain and receive physician orders as required.
Participate in on-call duties as defined by the on-call policy.
Perform other duties as assigned.
