About The Position

Puget Sound Home Health and Hospice is seeking an experienced and caring full-time Hospice Registered Nurse Case Manager (RN) in King County, Washington (Federal Way area) due to growth and the opening of new territories.

Requirements

Registered Nurse (RN) licensure
Ability to work independently and demonstrate critical thinking
Current BLS certification from AHA
Be a licensed driver with an insured automobile in good working order

Nice To Haves

Prior experience working in a hospice agency is strongly preferred

Responsibilities

Perform patient assessments (initial + ongoing)
Develop and update care plans
Administer meds and manage symptoms
Educate/support patients and families
Attend/participate in IDT meetings
Make home visits as needed
Document in EMR accurately and timely
Communicate with physicians and team
Triage urgent calls/on-call rotation
Guide on hospice philosophy/DNR/advance directives
Provide end-of-life emotional support
Mentor new staff
Ensure compliance with regulations
Coordinate with outside services (DME, pharmacy, etc.)
