About The Position

Elara Caring is seeking a compassionate Registered Nurse Case Manager to provide hospice care. The role assesses patients and families, monitors patient condition, communicates concerns to physicians and the care team, and educates and supports patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care. The nurse also provides education on hospice philosophy, pain management, and symptom management across care settings, and offers support at the time of death while completing bereavement assessments.

Requirements

• Hold a current state license as a Registered Nurse.
• Graduate from an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
• Have at least 1 year of experience as a nurse in a clinical care setting.
• Be able to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Have a valid driver's license and maintain an acceptable driving record, or have reliable transportation to travel between patient homes and assigned work locations.

Responsibilities

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and promptly communicate significant changes, concerns, or safety issues to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.
• Educate staff in nursing homes, hospitals, assisted living facilities, and other care settings on hospice philosophy, pain management, and symptom management.
• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.
