Registered Nurse Case Manager - Hospice
About The Position
Adoration Health is seeking a dedicated Hospice Registered Nurse Case Manager in Prattville, Alabama, serving Autauga, Elmore, and Montgomery counties. The role provides expert, patient-centered hospice care and support to patients, families, and caregivers. The schedule is Monday through Friday, 8 a.m. to 5 p.m., with participation in an on-call rotation as required by the local branch. The position offers opportunities to work independently with team support and flexible scheduling options. Adoration Health, an affiliate of BrightSpring Health Services, provides compassionate services in patients’ homes and supports patients, families, and caregivers. Requirements
- Graduate of an accredited nursing school.
- Current licensure to practice in the state of operation.
- At least one year of nursing experience.
- Knowledge of the hospice philosophy of care.
- Commitment to clinical and documentation excellence.
- Licensed driver with an automobile in good working order and insured according to state or organizational requirements.
Nice To Haves
- Three or more years of nursing experience preferred.
- Experience in hospice or a similar setting preferred.
Responsibilities
- Assess and monitor patients’ physical, emotional, and psychological needs.
- Create hospice care plans aligned with patients’ wishes and goals.
- Direct nursing care, including administering medications, treatments, and interventions.
- Provide pain and symptom management.
- Educate and support patients’ families and caregivers.
- Collaborate with the interdisciplinary team.
- Maintain accurate and timely documentation.
- Participate in the on-call rotation as required by the local branch.
