About The Position

Hospice of North Alabama is seeking a dedicated Registered Nurse Case Manager to provide patient-centered hospice care in Scottsboro, Alabama, covering Jackson County. The role provides care in patients’ homes and focuses on supporting patients and families during a critical time. The schedule is Monday through Friday, 8 a.m. to 5 p.m., with participation in an on-call rotation as required by the local branch. The nurse works independently while receiving team support and collaborates with an interdisciplinary team to deliver care tailored to patients’ wishes and goals.

Requirements

• Graduate of an accredited nursing school.
• Current licensure to practice in the state of operation.
• At least one year of nursing experience.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Licensed driver with an automobile that is insured according to state or organizational requirements and in good working order.
• Maintain current CPR certification in states where required.

Nice To Haves

• Three or more years of nursing experience preferred.
• Experience in hospice or a similar setting preferred.

Responsibilities

• Assess and monitor patients’ physical, emotional, and psychological needs.
• Create hospice care plans aligned with each patient’s wishes and goals.
• Direct nursing care, including administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patients’ families and caregivers.
• Collaborate with the interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in the on-call rotation as required by the local branch.
