About The Position

Crescent Hospice is seeking a full-time Hospice Registered Nurse Case Manager to provide compassionate end-of-life care throughout Charleston, Dorchester, and Berkeley County. The role delivers hands-on hospice nursing care, coordinates services, and guides patients and families throughout the hospice journey as part of an interdisciplinary care team. The schedule is Monday through Friday, 8:00 a.m. to 4:30 p.m., with occasional on-call duties. The position supports patient comfort, dignity, and quality of life while helping patients and families participate in personalized care planning.

Requirements

Hold a current, unencumbered Registered Nurse license.
Have at least one year of RN experience in a medical-surgical or acute-care setting.
Be proficient with computers, typing, and various programs, including electronic medical records (EMR).
Maintain current CPR certification if required by the state.
Have a current driver’s license, vehicle insurance, and access to dependable transportation.

Responsibilities

Respond to clinical referral information and address inquiries and requests compassionately.
Partner with the Clinical Director to assess client eligibility and suitability for hospice services.
Provide direct nursing care and support to patients, prioritizing comfort and quality of life.
Deliver nursing care using established principles and techniques to help ease pain and provide comfort.
Educate and advocate for patients and families, encouraging their participation in personalized care plans.
Share responsibility for on-call duties.
