About The Position

Sage Group HealthCare Staffing is seeking Hospice RNs for multiple travel/contract positions in Creve Coeur, MO. These positions are for a Hospice Registered Nurse Case Manager in a home setting. The contract length is 13 weeks, and both local and traveler candidates are accepted. The role involves providing and coordinating care for assigned Hospice patients, completing admissions, visits, and managing caseloads. The nurse will develop and implement nursing care plans, educate patients and families on disease management, and provide hospice nursing care within the scope of practice. Responsibilities include ensuring physician orders are transcribed correctly, assessing patient status, documenting care, and notifying physicians of significant changes. The territories covered are in the St. Louis area.

Requirements

  • Registered Nurse: 2 years - Required
  • Recent/Current Hospice nursing experience in a home health setting: 1 year within the last 3 years – Required
  • Hands on Home Care Home Base (HCHB) charting experience – Required
  • Must have valid Driver’s License and Personal Auto Insurance
  • Must be Covid Vaccinated or provide medical or religious exemption
  • ASN or an equivalent – Required
  • RN License for State applying or Compact if applicable - Required
  • BLS Certification from AHA or American Red Cross - Required

Nice To Haves

  • BSN or an equivalent – Highly preferred

Responsibilities

  • Provide and coordinate care for assigned Hospice patients
  • Complete admissions, visits and manage assigned caseload
  • Develop and implement nursing care plans for patients as assigned
  • Empower patients and families to reach healthcare goals by educating them on disease management
  • Provide Hospice nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures
  • Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure
  • Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented
  • Notify physicians of significant status changes
  • Document and complete charting
