About The Position

Sage Group HealthCare Staffing is seeking a Registered Nurse Hospice Case Manager for a 13-week travel/contract position in Bedford, NH. This role involves providing and coordinating care for assigned hospice patients in their homes, developing and implementing nursing care plans, and performing case management. The position requires a valid Driver's License, personal auto insurance, and COVID vaccination or an exemption. The nurse will be responsible for assessing patients, documenting visits, and providing bereavement support to families.

Requirements

  • Registered Nurse: 2 years - Required
  • Experience with Hospice nursing in a home setting: 1 year within the last 3 years - Required
  • Home Care Home Base (HCHB) charting experience - Required
  • Must be able to do admissions - Required
  • Must have valid Driver’s License and Personal Auto Insurance
  • Must be Covid Vaccinated or provide medical or religious exemption
  • ASN or an equivalent – Required
  • RN License for State applying or Compact if accepted - Required
  • BLS Certification from AHA or American Red Cross - Required

Nice To Haves

  • Case Management experience - Highly preferred
  • BSN or an equivalent – Highly preferred

Responsibilities

  • Provide and coordinate care for assigned hospice patients in the comfort of their homes.
  • Develop and implement nursing care plans, including Start of Care and admissions, as assigned.
  • Case management and provide clinical oversight to interdisciplinary teams.
  • Provide nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
  • Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
  • Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
  • Notify physicians of significant changes in status.
  • Document and complete charting for all patient visits within 24 hours.
  • Provide Bereavement Support to patient families.

Benefits

  • $2200 Weekly Gross pay based on a 40-hour work week (Includes Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)
  • Mileage Reimbursement: $0.54/mile for distance covered between patient home appointments
