About The Position

Sage Group HealthCare Staffing is seeking a Hospice Registered Nurse for 13-week travel/contract positions in Anchorage, AK. We are accepting both local and traveler candidates. This role involves providing and coordinating care for assigned hospice patients in their homes, developing and implementing nursing care plans, supporting patient comfort and pain management, and performing case management. The nurse will provide nursing care and services within the standards and scope of nursing practice, ensure physicians' orders are transcribed appropriately, assess patients' status ongoingly, document all patient visits within 24 hours, and notify physicians of significant changes in status. The territory to cover is 40 miles north and 40 miles south of Anchorage.

Requirements

• Registered Nurse: 2 years - Required
• Hospice RN experience in a home setting: 1 year within the last 3 years - Required
• EMR: Home Care Home Base (HCHB) charting
• Must have valid Driver’s License and Personal Auto Insurance
• ASN or an equivalent – Required
• RN License for State of the state applying or Compact if applicable - Required
• BLS Certification from AHA or American Red Cross - Required

Nice To Haves

• BSN or an equivalent – Highly preferred

Responsibilities

• Provide and coordinate care for assigned hospice patients in comfort of their homes.
• Develop and implement nursing care plans for all types of visits including Start of Care and admissions as assigned.
• Support patient comfort and pain management.
• Case management and provide clinical oversight to interdisciplinary team.
• Provide nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
• Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
• Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
• Notify physicians of significant changes in status.
• Document and complete charting for all patient visits within 24 hours.
