About The Position
Calvary Hospital and Calvary @Home provide palliative care to advanced cancer and other terminally ill patients and their families. The Hospice Nurse Liaison coordinates applications through the admission review process, maintains relationships with referring institutions and agencies, supports development and marketing of Calvary Hospital’s Home Care and Hospice Services, and provides pre-admission assistance to patients, families, and referrers.
Requirements
- Current New York State Registered Nurse license or permit.
- Minimum of three years of relevant clinical experience.
- Working knowledge of inpatient, outpatient, home care, and hospice settings; admission and continued-stay criteria; and third-party payers.
- Must have a personal car to conduct site assessments.
Nice To Haves
- Current valid New York State driver’s license.
- Bachelor’s degree.
Responsibilities
- Coordinate applications through the admission review process.
- Coordinate and maintain relationships with referrers at assigned institutions and agencies.
- Participate in the development and marketing of Calvary Hospital’s Home Care and Hospice Services.
- Provide pre-admission assistance to patients, families, and referrers.
