Per diem Hospice Nurse Case Manager
About The Position
Mission Hospice & Home Care is a local, independent nonprofit serving patients and families in the San Francisco Peninsula and South Bay with quality care and compassionate end-of-life support and education. Founded in 1979, the organization provides physical, emotional, and spiritual support from a life-threatening diagnosis through death and bereavement. The Hospice Nurse Case Manager is responsible for an assigned caseload or facility, providing skilled nursing assessment, planning, and care to maximize patient and family comfort and health. This per diem role is based in San Mateo and serves San Mateo County, with additional coverage in Santa Clara County as needed. The schedule is Sunday, Thursday, and Friday, 8:30 a.m. to 5:00 p.m. The position offers competitive pay and a supportive work environment. Requirements
- One year of experience as a professional nurse in the past three years, or a Bachelor’s degree in Nursing from an accredited School of Nursing.
- Current license as a registered professional nurse in the State of California.
Nice To Haves
- Experience in a home health and/or hospice setting is preferred.
- Public Health Nursing certification is highly desirable.
- HPNA certification is preferred.
Responsibilities
- Assume primary responsibility for a patient and family, using the nursing process to assess, plan, implement, and evaluate their needs, goals, and interventions.
- Coordinate patient and family services and prioritize needs with members of the transdisciplinary team.
- Use a case management approach and refer patients and families to other services as needed.
- Coordinate implementation of the plan of care and ensure care coordination and case management for each patient.
- As applicable, apply criteria for hospice admission and recertification to establish appropriate levels of care and patient eligibility.
