Assisted is seeking a per diem RN Case Manager to support its Santa Barbara Hospice branch. The role involves assessing hospice patients’ needs, providing specialized nursing services and treatments, initiating preventative and rehabilitative nursing procedures, and supporting patients, families, and caregivers. Requirements

• Graduate of an approved school of professional nursing.
• Current, active license with the California Board of Nursing.
• Current CPR.
• Health clearance.
• TB clearance.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients’ needs.
• Provide services and treatments requiring substantial and specialized nursing skills.
• Initiate appropriate preventative and rehabilitative nursing procedures.
• Counsel, support, and instruct patients, families, and caregivers in meeting nursing and related needs.
